AFC Teens Conference 2023 - Our World Transformed (Senior Grades 9-12)

PLEASE READ THE WAIVER CAREFULLY BEFORE PROCEEDING - By proceeding and completing the TC 2023 Registration, you agree to the following terms and conditions below:

As Participant:

1. I agree to follow all rules set forth by Ambassadors For Christ in Canada ("AFC").
2. I agree to be responsible for obtaining the appropriate equipment needed to participate in TC 2023.
3. I agree that a phone call will be made to TC by the parent/guardian if I have to leave between the programs on any day of Teens Conference 2023 ("TC 2023").
4. I agree that I fully accept all the responsibilities for any injury to myself while I am participating in TC 2023.
5. I agree that in the event of an accident or sickness while online/in-person, AFC, its officials, staff, and volunteers are hereby released from any and all liability.

As Parent/Guardian:

1. I agree to follow all rules set forth by Ambassadors For Christ in Canada ("AFC").
2. I give permission to my child to participate in TC 2023.
3. I agree that a phone call will be made to TC by parent/guardian if my child has to leave between the programs on any day of Teens Conference 2023 ("TC 2023"). (Phone number will be provided in the confirmation email.)
4. I agree that in the event of an accident or sickness while online/in-person, AFC, its officials, staff, and volunteers are hereby released from any liability.
5. I agree that in the event my child conducts himself/herself in a manner that is disruptive, he/she will be dealt with appropriately, including the possibility of contacting me and/or removing him/her from participating.

PHOTO RELEASE WAIVER

I agree and acknowledge that pictures and videos of my child may be taken during the event and used for future promotional purposes.
